ELIGIBILITY

Permanent, full-time employees will become eligible for coverage:

- on the Plan Effective Date, if employment commenced on or prior to the Plan Effective Date.

- following 1 month of continuous, active employment if employment commenced after the Plan Effective Date for Class A, Class 005 and Class 006.

- following 6 months of continuous, active employment if employment commenced after the Plan Effective Date for Class B, Class 001, Class 002, Class 003, Class 004, Class 007, and Class 008.

Class A - FUNSCAD Unit I, Excluded and NSGEU
Class B - FUNSCAD Unit II

Retired employees are not eligible for coverage.

SUMMARY OF BENEFITS

BASIC LIFE INSURANCE (Class A)

You are eligible for an amount of insurance equal to 2.5 times annual earnings,  subject to a maximum benefit of $ 300,000.

No-Evidence Limit: $300,000.

Coverage terminates on the date you attain age 65 or retirement, whichever is earlier, and as outlined in the General Provisions section.

OPTIONAL LIFE INSURANCE (Class A)

If you are under age 65, you may apply for this coverage:
 1) for yourself;
 2) for your spouse, provided he/she is under age 65; or
 3) for both you and your spouse;

Coverage is available in units of $ 10,000, subject to a maximum benefit of $ 300,000.

Evidence of insurability, satisfactory to the Insurer, shall be required for amounts in excess of $50,000.  This coverage is available in addition to, not in lieu of, Basic Life and/or Basic Dependent Life Insurance.  Coverage terminates on the date you attain age 65 or retirement, whichever is earlier, and as outlined in the General Provisions section.  Coverage for your spouse terminates on the date you attain age 65 or retirement, or your spouse attains age 65, whichever is earlier.

BASIC DEPENDENT LIFE INSURANCE (Class A)

Spouse  $ 5,000

Each Child $ 2,500

Coverage terminates on the date your life insurance terminates, and as outlined in the General Provisions section.

BASIC LIFE INSURANCE (Class B)

You are eligible for an amount of insurance equal to 2.5 times annual earnings, subject to a maximum benefit of $ 300,000.

No-Evidence Limit: $300,000.

Coverage terminates on the date you attain age 65 or retirement, whichever is earlier, and as outlined in the General Provisions section.

OPTIONAL LIFE INSURANCE (Class B)

If you are under age 65, you may apply for this coverage:
 for yourself;
 for your spouse, provided he/she is under age 65; or
 for both you and your spouse;

Coverage is available in units of $ 10,000, subject to a maximum benefit of $ 300,000.

Evidence of insurability, satisfactory to the Insurer, shall be required for amounts in excess of $50,000.  This coverage is available in addition to, not in lieu of, Basic Life and/or Basic Dependent Life Insurance.  Coverage terminates on the date you attain age 65 or retirement, whichever is earlier, and as outlined in the General Provisions section.  Coverage for your spouse terminates on the date you attain age 65 or retirement, or your spouse attains age 65, whichever is earlier.

BASIC DEPENDENT LIFE INSURANCE (Class B)

Spouse   $ 5,000
Each Child  $ 2,500

Coverage terminates on the date your life insurance terminates, and as outlined in the General Provisions section.

EMPLOYEE BASIC LIFE INSURANCE

In the event of your death while insured, the amount of your Life Insurance is payable to your beneficiary.  You may change your beneficiary at any time by written notice to your Employer, subject to any policy or legal limitations.

WAIVER OF PREMIUM FOR DISABILITY

If you become totally disabled for 98 consecutive months before age 65, your Life Insurance will be continued free of charge until you cease to be totally disabled or you reach age 65, whichever occurs first.

To qualify, for the first 98 consecutive months of waiver of premium, you must be unable to perform any and every duty of the your occupation or employment.  After such 98 months, Totally Disabled shall mean you are not able to perform any and every duty of any occupation or employment for which you are reasonably qualified by education, training or experience.  Such incapacity must result from a medically determinable physical or mental impairment.

Note:  In order to qualify for the Waiver of Premium benefit you must notify Maritime Life of your disability within one (1) year of your last active day at work, and must furnish proof of your disability satisfactory to the Insurer within 18 months of that last active working day.

CONVERSION PRIVILEGE

Your Life Insurance continues for 31 days following either the termination of your employment, or your classification changing to one in which you are not insured.  During this 31 day period you may convert the amount of your Group Life Insurance, provided you are under 65 years of age, to any individual whole life or convertible one-year term or term to age 65 plan without submitting evidence of health.

The amount of the individual policy shall not exceed the amount of insurance for which you were insured when coverage was dis¬continued, subject to a maximum of $200,000 (Basic and Optional insurance combined) less any amount you become eligible for under a replacing contract of group life insurance.

The premium rate will be determined from your age and class of risk at the time of conversion.

Note:  The conversion privilege does not apply for loss of insurance as a result of:
i) any age reduction specified in the Summary of Benefits; or
ii) if insurance terminates when you reach the age specified in the Summary of Benefits of Insurance section or upon your retirement.

EMPLOYEE OPTIONAL LIFE INSURANCE

In the event of your death while insured, the amount of your Optional Life Insurance is payable to your beneficiary.

However, if you die due to self-destruction while sane or insane, the Insurer will not pay any part of Optional Life Insurance which became effective less than two years prior to your date of death.  Misstatement of non-smoker status by you shall constitute fraud, and the Insurer will not pay any part of your Optional Life Insurance, regardless of the cause of death.

The provisions of this coverage shall be the same as for Basic Life Insurance.

DEPENDENT BASIC LIFE INSURANCE

In the event of the death of your spouse and/or dependent children while insured, the amount of Dependent Life Insurance is payable to you.

WAIVER OF PREMIUM FOR DISABILITY

If you become totally disabled for 98 consecutive months before age 65, Dependent Life Insurance for your spouse will be continued free of charge until you cease to be totally disabled, you reach age 65 or the policy or coverage terminates, whichever occurs first.

SPOUSE OPTIONAL LIFE INSURANCE

In the event of the death of your spouse while insured, the amount of your spouse's Optional Life Insurance is payable to you.

However, if your spouse dies due to self-destruction while sane or insane, the Insurer will not pay any part of your spouse's Optional Life Insurance which became effective less than two years prior to the  date of death.  Misstatement of non-smoker status by you or your spouse shall constitute fraud, and the Insurer will not pay any part of the Optional Life Insurance, regardless of the cause of death.

GENERAL PROVISIONS

WHEN YOUR INSURANCE STARTS

Your insurance comes into effect on the latest of the following dates if you are actively at work on that date.

  the date you become eligible;
  the date you apply for coverage for yourself and/or your family; or
  if Evidence of Insurability is required the date it is approved by the Insurer.

EVIDENCE OF INSURABILITY

Evidence of Insurability is required if:

  you apply for insurance more than 31 days after becoming eligible to apply;
  you reapply after your insurance has terminated due to non-payment of premium; or
  you apply for Optional Life Insurance for yourself and/or your spouse; or
  your amount of insurance exceeds or increases beyond the No-Evidence Limit.

WHEN YOUR INSURANCE TERMINATES

Your insurance terminates on the earliest of the following:

  non-payment of premium;
  a change in your classification to one not insured;
  termination of your employment;
  termination or amendment of the Master Policy;
  your commencing active duty in any armed forces; or
  the date outlined in the Summary of Benefits.

Note:  In the event you are absent from work due to sickness, injury, layoff or leave of absence, your insurance coverages may continue for a period as outlined in the Master Policy, but only if the required premiums are paid.

Coverage for Emergency Travel Assistance and Outside Canada Expenses under Supplementary Health Expense is not available during a leave of absence.
